The Waiver of Liability and Emergency Medical Information Forms are required to be filled out and turned in by all volunteers before you can begin work.

Parental Authorization for Treatment of Minor Child and Release and Waiver of Liability for Minors

Required: All Minor Volunteers

This form is required for all minors and must be signed by a custodial parent or guardian and notarized by a notary public. No exceptions.
