Fully completed applications and all supporting documents must be received in our office no later than November 2nd, 2026.
This application is for those that attended our live courses on September 21, 22 or 29th. If you did not attend the live courses please call our office (865) 483-5433 option 3 to discuss other options.
- Download and Print the “Application” and “Required Supporting Documents” pages.
- Fill them out using a pen, attach them with all required supporting documents (copies only, please no originals)
- Mail or drop them off to our office at:

You may also pick up blank applications up at our office (address above) Monday – Friday 9:30 till 5:00
Incomplete applications or applications without supporting documentation will not be considered.
Our office will then contact you and let you know if your application has been reviewed or needs further documentation.
